Certified Home Health Aides & Homemakers
The hospice team is composed of many functions and personalities. But the home health aides and the homemakers are the ones who will be spending the most time with the patients and their families. Their task involves providing care for patients at home and assisting them as they perform daily activities and even light chores if possible. They do not only provide assistance but also empower the patients to live a dignified life by allowing them to be as independent as they can but providing them assistance on feeding, grooming, eating, and other things. During regular visits, the HHA/Homemaker will check the vital signs and report them to the medical team.
The home health aide shall ensure the safety of the patient while they are at home and shall make sure that the patient and her environment is safe, neat, and clean. HHAs may be tasked to do errands for the family and shall provide respite for the family member who have been providing care for the patient.
MISSION STATEMENT
To provide hospice services that are designed to provide pallative care, alleviate the physical, emotional, social and spiritual discomforts of an individual who is experiencing the last phase of life due to existence of terminal disease and provide supportive care to the primary care to the primary caregiver and the family of the patients.
